*Prepaid membership credits are exclusively for use by company employees and are non-transferable. These credits can be redeemed at all Hydrate IV Bar® locations nationwide. The company commits to maintaining a minimum of 5 memberships for at least 6 months per membership, transitioning to a month-to-month basis thereafter unless canceled by a company representative. Unused IVs or B12 injections remain valid for 6 months from the payment date for active memberships with no refunds provided for unused services. Additional services and retail purchases must be made individually by employees. Company representatives are responsible for notifying Hydrate IV Bar® of any changes in employee status for membership updates or cancellations. Credit allocation per individual is determined by the company and communicated to employees. Corporate memberships are subject to Hydrate’s policies regarding cancellations, no-shows, and late arrivals, with the right reserved to cancel or pause memberships under certain circumstances. The company authorizes Hydrate to charge the provided credit card monthly, with membership suspension occurring after one failed autopay and termination after three failed autopays, with any remaining obligations fulfilled monetarily.
